  • Some of the surrounding behavioural health clinic's weren't able to accomodate our patient's mental health needs. This was either because they didn't take the patient's insurance, or there was too long of a waitlist to be seen. Also, when patients would score high on a depression or anxiety screening, it's hard to capture every single patient that needed to be referred to a behavioural health partner solution.

  • MHT created Alerts associated with assessments where if a patient scored above a cetain threshhold, the referral clinic would have a care coordinator who would be alerted of the patient scoring high. They also connected us to a TelePsych partner (LinqCare) that was able to accomodate our patient's mental health needs. Alert's were set up and patient's needs were taken care of in a timely fashion.

  • No patient slipped through the cracks. The care coordinator was able to capture every patient needing a higher level of care through a simple automated email. Patient's also recieved mental health care quicker this way then before.

  • Identifying patients who are struggling with anxiety & depression and then getting them the help that they need right away.

  • Using MHT and LinqCare.

    BMC admins send out assessments to patients when they check in. Once it's completed the MAs review their scores & enter them into the EMR so the clinicians have that information when they see the patient. LinqCare care coordinators recieve an alert if/when a patient scores a 15 or above on the GAD-7 and PHQ-9. Once they have the alert, they have consent to contact the patient & follow up to set up an appointment. Example: Linqcare coordinator received 18 referrals for the month of May, 12 of those referrals made appointments.

  • Identifying high risk patients & getting them the care that they need as soon as possible. Using MHT in a primary care setting helps provide better care for the patient by evaluating their overall wellbeing - by partnering with MHT & LinqCare, BMC is able to screen patients for anxiety & depression

The MHT platform is easy to use and the MHT team has been great to work with. We also are now qualifying those patients in need to receive assistance from our telepsych partner as well.
